[ESP | ENG] Hemotórax: tratamiento según estado de gravedad del paciente. / Hemothorax: treatment according to the severity of the patient

Cuando un paciente presenta hemotórax es fundamental el saber qué tipo de tratamiento requiere según su estado de gravedad, así mismo requiere de valoración por parte del personal médico y de enfermería.

When a patient presents hemothorax, it is essential to know what type of treatment is required according to the state of severity of it, it also requires assessment by the medical and nursing staff.

Se puede diferenciar la gravedad según el tipo de hemotórax, según sea agudo o masivo.

The severity can be differentiated according to the type of hemothorax, depending on whether it is acute or massive.

Un paciente con hemotórax agudo requiere del manejo de cualquier situación que pueda comprometer su vida, esto abarca el controlar el sangrado para mantener el estado hemodinámico estable. Se considera que están hemodinámicamente estables cuando el volumen sanguíneo es menor o igual a 300 cc. En éstos casos se opta por mantener al paciente bajo vigilancia radiológica y clínica.

A patient with acute hemothorax requires the management of any situation that could compromise her life, this includes controlling bleeding to maintain a stable hemodynamic state. They are considered to be hemodynamically stable when the blood volume is less than or equal to 300 cc. In these cases, it is decided to keep the patient under radiological and clinical surveillance.

Por otro lado, si el paciente presenta ya sea inestabilidad hemodinámica o un volumen sanguíneo mayor de 300 cc requiere de drenaje pleural, en ésta ocasión deberá actuar el personal médico de cirugía especializado en tórax. En éste tratamiento el paciente tendrá dentro de su cavidad pleural una sonda que permitirá drenar la hemorragia.

On the other hand, if the patient presents either hemodynamic instability or a blood volume greater than 300 cc requires pleural drainage, on this occasion the medical surgical personnel specialized in the thorax should act. In this treatment, the patient will have a probe inside his pleural cavity that will allow the bleeding to drain.

En caso de hemotórax masivo se requiere de intervención quirúrgica, donde el volumen sanguíneo es igual o mayor a 1.5 litros que se manifiestan de forma inmediata luego de la colocación de la sonda dentro de la cavidad pleural pulmonar.

In the case of massive hemothorax, surgical intervention is required, where the blood volume is equal to or greater than 1.5 liters that manifest immediately after the placement of the probe inside the pulmonary pleural cavity.

Es importante saber que éstos pacientes requieren atención inmediata debido a las graves complicaciones que trae la misma patología, con un diagnóstico acertado bajo la valoración de su estado hemodinámico es posible lograr una satisfactoria recuperación.

It is important to know that these patients require immediate attention due to the serious complications that the same pathology brings, with a correct diagnosis under the evaluation of their hemodynamic status it is possible to achieve a satisfactory recovery.
